REFERRAL PROCESS

We are currently accepting new patients. A new patient intake request by the patient or by provider can be processed either by fax or phone. SNBCare team processes new patient’s intake within 48 hours after receiving a fax or call.

To request online:
      1.  Please download or click the button below to complete the New Appointment Request Form
      2.  If faxing a referral, please fax the New Appointment Request Form to (781) 666-2712.
      3. To ensure timely scheduling and coordination of care, please ensure all information requested on the New Appointment Request Form is included and accurate.
      4. Our intake staff will review the request and contact the patient within 48 hours to complete the intake process and schedule any appointments.
      5. For TMS referrals or those interested in learning more about TMS, please complete this form:  PHQ9
To request by phone:

Call on (781)666–2711. Your call will be processed by a service representative in the Intake Office, who will collect brief demographic and clinical information about your patient. The intake staff may request that you fax additional clinical information so they can triage and prepare for the appointment appropriately.
